In order to help process an insurance claim or meet other obligations, fire administration personnel can assist you in obtaining an ambulance or fire report.  Please contact the department with your request at your earliest convenience and we will assist you through the process.  A fire report may be obtained at the Fire Administration Building located at 500 Southwestern Boulevard or by calling 972-304-3512.  Due to HIPPA, a request for an ambulance report must be made in person with proper picture identification, or by completing the appropriate notarized Medical Authorization form available below and submitting it to our office located at 500 Southwestern Boulevard.  Our normal business hours are from 8 a.m. until 5 p.m., Monday through Friday.  The appropriate documentation must be completed prior to the release of an ambulance report.
